The Care Now Plan 

The Care Now Plan includes a 10% service fee, which is applied to the total cost of services. This fee supports the ability to offer flexible, pay-over-time access to care.

You agree to pay the full plan total, including the service fee, according to the selected payment schedule. A copy of your schedule will be available for your reference.

Payment Method

You confirm that your payment method will remain active, valid, and able to process scheduled payments for the duration of the plan.

If a payment does not process successfully, you agree to provide an updated payment method so the plan can continue.

Care & Service Continuity

Care is delivered based on the expectation that payment plans remain active.

If payments fall behind, future services may be paused until the account is brought current. Any services already provided are non-refundable.

Changes & Support

We understand that circumstances can change.

If you need to adjust your plan, please contact our team. We will make reasonable efforts to work with you whenever possible.

Cancellation

If the Care Now Plan is cancelled before completion, any remaining balance becomes due.

Commitment to Completion

By enrolling, you are committing to complete the full payment plan. Early payoff is available at any time.
